Adult Legal Caretaker Information

Adult legal caretakers from school districts or other institutions will be asked to confirm that they have signed and dated youth activity permission, photo, and release forms according to their organizational policies and procedures. Caretakers are also asked to confirm that the families of the youth attendees have been informed, and have consented to the face, likeness, voice, and appearance of the youth attendee to be included in Summit recordings, photos and/or promotional materials. These materials are documentation of the Summit experience, and consent to be included is done without compensation, reservation or limitation.

I also confirm that the families of all youth included in this registration form have been informed and have consented to the inclusion of the face, likeness, voice, and appearance of their youth attendee to be included in Summit recordings, photos and/or promotional materials as part of the documentation of the Summit experience without compensation, reservation or limitation.
